Learn what they are, why they happen, and why urgent diagnosis and treatment are needed to <strong>prevent a stroke<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">What Are Amaurosis Fugax and rTIA?<\/h2>\n\n\n\n<p class=\"wp-block-paragraph translation-block\"><strong>Amaurosis fugax (AFx)<\/strong> is a <strong>transient monocular vision loss<\/strong> caused by <strong>temporary retinal ischemia<\/strong>. This means that blood flow to the retina drops suddenly and reversibly, often because of <strong>embolic events<\/strong> or hemodynamic changes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph translation-block\"><strong>rTIA<\/strong> is a more specific term for this <strong>momentary vision loss<\/strong> resulting from transient retinal ischemia. The retina is highly sensitive to oxygen deprivation because of its high metabolic demand, which leads to this temporary vision loss.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Main Causes of Retinal Ischemia<\/h3>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">The most common cause of transient retinal ischemia is <strong>thromboembolic events<\/strong>. Small fragments (cholesterol, fibrin-platelet, or calcium emboli) usually break off from <strong>atherosclerotic plaques<\/strong> in the <strong>ipsilateral carotid artery<\/strong>, the heart, or the aortic arch, and travel until they temporarily block the retinal arteries.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Other less common causes include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"translation-block\"><strong>Retinal artery vasospasm<\/strong><\/li>\n\n\n\n<li class=\"translation-block\"><strong>Arterial dissection<\/strong> (such as carotid artery dissection)<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Vasculitis<\/strong> (especially <strong>giant cell arteritis<\/strong>)<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Hypercoagulable states<\/strong><\/li>\n\n\n\n<li>In rare cases, transient increases in intraocular pressure or external ocular compression.<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Characteristic Symptoms and How to Diagnose<\/h2>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">The most distinctive symptom of AFx\/rTIA is <strong>transient monocular vision loss (TMVL)<\/strong>, often described as a \"curtain\" or \"shadow\" descending or rising across the visual field of the affected eye. The vision loss is <strong>sudden<\/strong> and lasts from seconds to several minutes, almost always resolving in less than an hour. If it lasts longer, it could indicate a <strong>retinal artery occlusion<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">The <strong>diagnosis of AFx\/rTIA<\/strong> is based on:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"translation-block\">A <strong>detailed medical history<\/strong>: It is crucial to ask about the nature of the visual loss, associated symptoms, and <strong>risk factors for vascular disease<\/strong> (such as hypertension, diabetes, or smoking).<\/li>\n\n\n\n<li class=\"translation-block\">A <strong>complete physical examination<\/strong>: This includes a thorough neurological assessment to rule out persistent deficits. This risk can be 3% to 10% in the first two days and 9% to 17% in the first 90 days after the event, a risk similar to that of TIAs affecting the central nervous system (CNS).<\/p>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">Risk scores such as <strong>ABCD2<\/strong> (and its modifications ABCD3 and ABCD-I) are used to assess stroke risk after a TIA.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Management and Treatment: Aggressive Stroke Prevention<\/h2>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">Management of rTIA\/AFx is similar to that of cerebral TIAs, focusing on <strong>stroke prevention<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">An <strong>urgent diagnostic workup<\/strong> is recommended to identify underlying causes that can be addressed. This includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"translation-block\"><strong>Brain and vascular imaging<\/strong>: Such as carotid Doppler ultrasound and CT or MR angiography.<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Cardiac evaluation<\/strong>: Electrocardiogram (ECG) and echocardiogram.<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Laboratory tests<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph translation-block\"><strong>Medical treatment<\/strong> commonly includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"translation-block\"><strong>Antiplatelet therapy<\/strong>: Aspirin or clopidogrel to reduce the risk of new ischemic events.<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Anticoagulation<\/strong>: Indicated in cases of cardioembolic sources, such as <strong>atrial fibrillation<\/strong>.<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph translation-block\"><strong>Risk factor modification<\/strong> is vital: controlling <strong>hypertension<\/strong>, <strong>diabetes<\/strong>, and <strong>hyperlipidemia<\/strong>, and <strong>quitting smoking<\/strong>. Lifestyle changes, such as a healthy diet and regular exercise, are also advised.<\/p>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">In patients with <strong>significant carotid stenosis<\/strong>, <strong>carotid endarterectomy<\/strong> or <strong>carotid stenting<\/strong> may be considered to reduce stroke risk. It is recommended that all patients with TIA\/AFx receive antiplatelet therapy before a more complete evaluation. In high-risk patients, <strong>short-term dual antiplatelet therapy<\/strong> (aspirin and clopidogrel) may be considered.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">The Critical Role of the Ophthalmologist in Diagnosis<\/h2>\n\n\n\n<p class=\"wp-block-paragraph translation-block\"><strong>Ophthalmologists<\/strong> are essential in the <strong>early recognition<\/strong> and appropriate management of rTIA.
