This SamurEye episode takes a deep dive into the radical but effective strategy we used in an extreme case of <strong>acute angle closure<\/strong> in an 18-year-old patient with an axial length of only <strong>16.75 mm<\/strong>.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading enfoca-tam-h3\">The Crisis and the Triple Approach<\/h2>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">The patient was referred in crisis, with an <strong>IOP of 40 mmHg<\/strong> that did not respond to medical treatment. The characteristics of such a small eye demand going beyond standard procedures. This is where the combined single-session strategy comes in, proving superior to multiple re-operations during a crisis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph translation-block\"><strong>The Definitive Approach:<\/strong><\/p>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li class=\"translation-block\"><strong>Lensectomy:<\/strong> Removal of the crystalline lens, a key component that takes up space and pushes the iris diaphragm forward.<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Posterior vitrectomy:<\/strong> Crucial for removing the <strong>posterior vitreous component<\/strong> that exerts pressure and promotes angle closure. This creates vital space in the anterior chamber.<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Transscleral cyclophotocoagulation (CPC):<\/strong> Controlled destruction of part of the ciliary body. This maneuver reduces aqueous humor production, achieving immediate pressure control.<\/li>\n<\/ol>\n\n\n\n<h2 class=\"wp-block-heading enfoca-tam-h3\">Clinical Lessons and Challenges of the Nanophthalmic Eye<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">The main challenge in these cases is not only controlling pressure but avoiding damage to already compromised eye tissue.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li class=\"translation-block\"><strong>Mannitol as a Critical Ally:<\/strong> It was confirmed that giving <strong>preoperative mannitol<\/strong> (limited to 1 <strong>hour<\/strong>) is essential. It dehydrates the lens and makes surgery easier without causing complications in the capsulorhexis.<\/li>\n\n\n\n<li class=\"translation-block\"><strong>Goodbye to Serial Aggression:<\/strong> Comparing the patient's two eyes, the second eye, treated with the single-session triple strategy and reduced <strong>CPC<\/strong> parameters, had a cleaner recovery. The first eye, operated on during the crisis and re-operated, developed areas of <strong>scleromalacia<\/strong> due to active inflammation.<\/li>\n\n\n\n<li class=\"translation-block\"><strong>A War of Attrition, Not One-Off Efforts:<\/strong> The ultimate goal is <strong>preservation<\/strong>. By choosing <strong>CPC<\/strong> and <strong>vitrectomy<\/strong>, we avoided touching the young patient's conjunctiva and sclera. This leaves the \"battlefield\" intact for more aggressive filtering procedures (valves or trabeculectomies) the patient may need in the decades ahead.<\/li>\n<\/ul>\n\n\n\n<h2 class=\"wp-block-heading enfoca-tam-h3\">Takeaways for Your Practice<\/h2>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">Managing <strong>nanophthalmic glaucoma<\/strong> demands a proactive approach.
